Health care professionals face everyday people in pain. The scientific interest toward this topic, even if it is very important, has been marginal during large part of the scientific approach to medical problems. Only in the last part of the previous century, some scientists have focused their attention on the suffering patients. The physiological studies on the mechanisms of pain perception have evolved together with the developments of technology, but still there is much to do. In fact, just a bit more than a year ago the Noble prize for Medicine has been assigned to 2 scientists for their studies on peripheral receptors involved in pain perception.  
The real clinical interest toward pain patients has increased with the study on the epidemiology of pain. These studies have shown the huge diffusion of the phenomenon, at many levels. Incidence and prevalence of acute pain, in different clinical settings have been evaluated, but still many data are missing. Chronic pain is represented by a multifaceted phenomenon. Hence, the epidemiology related to this specific type of pain is still very confusing and would deserve many more studies and data.  
This Special Issue will explore to address the new challenges of pain epidemiology, and how this may help clinicians to better understand their patients and the necessary managements. We hope this would help to attract the attention of politicians and health care providers, and contribute to increase the optimal managements for the suffering patients.
